The Better Numbers Cardiometabolic Health Program

An evidence-based coaching program designed for adults who want to take a more proactive, structured approach to improving their A1C, cholesterol (lipid profile), blood pressure, and/or weight. Better Numbers isn’t just for those who are “stuck”—it’s for anyone who wants a deeper understanding of their cardiometabolic health, a highly personalized strategy, and closer support than insurance sessions alone provide.

Through personalized nutrition and lifestyle coaching, we’ll bring the different pieces of your health into focus—looking beyond individual numbers to understand the patterns and factors that may be influencing them. During this intensive phase, we’ll translate those insights into targeted strategies, put them into practice, and refine them based on your response. The goal is to give you a strong foundation for meaningful, measurable improvements in your numbers over time—and the understanding, confidence, and practical tools to keep moving in that direction.

When clinically appropriate, the program includes structured continuous glucose monitoring (CGM) assessment and interpretation, integrating glucose patterns with your food intake and other relevant lifestyle data to identify individualized patterns and guide targeted changes.

What’s Included

Your initial 90-minute consultation provides a comprehensive review of your nutrition, lifestyle, labs, and other relevant health data to identify patterns, priorities, and opportunities for targeted change. From that foundation, the six-week intensive program adds a deeper level of analysis, personalization, and support as we put your plan into practice and refine it based on what we learn.

In addition to five 55-minute follow-up sessions over six weeks (billed separately to insurance when covered), the program provides:

  • Ongoing analysis of your nutrition, lifestyle, labs, and other relevant health data as new information becomes available, helping us identify patterns, evaluate your response, and refine your strategies throughout the program.

  • Advanced case research and analysis when your history, symptoms, lab results, or other health data warrant a deeper look—so recommendations are informed by the evidence and specific to your situation.

  • A personalized nutrition and lifestyle strategy that evolves throughout the program as we learn more about your needs, responses, preferences, and real-life barriers.

  • Your personal coaching Roadmap and individualized progress-tracking tools to help us focus on the behaviors and outcomes most relevant to your goals and see what is changing over time.

  • Personalized tools, resources, and educational materials created or selected for you as needs arise throughout the program—not a one-size-fits-all library of information.

  • Personalized meal planning to translate your nutrition targets into meals and eating patterns that fit your preferences, schedule, cultural needs, and daily life.

  • Supplement review and personalized recommendations, including evaluation of current supplements and development or refinement of a supplement protocol when appropriate.

  • Continuous glucose monitoring (CGM) assessment and interpretation, when clinically appropriate, integrating glucose patterns with food intake and other relevant lifestyle data to identify your individual responses and guide targeted strategies.

  • Between-session support through your private portal for questions, updates, and troubleshooting, with a response within 48 hours, so we can address issues as they arise rather than waiting for your next appointment.

  • Priority scheduling, including access to appointment times not available to non-program clients.

  • Progress summary for your PCP or other healthcare provider, if requested, to support coordination of your care.

Why This Program is Different

Insurance-covered sessions give us valuable time to assess your progress, work through challenges, and determine next steps. But highly personalized care often requires substantial work outside of those appointments—time to investigate a clinical question, examine patterns across multiple days of dietary or health data, connect findings across labs and other measures, research the evidence, and translate what we learn into recommendations and tools specific to you.

The Better Numbers program creates dedicated time for that deeper work while also giving us a more continuous six-week period to implement, evaluate, and refine your strategy. Rather than waiting until each appointment to decide what comes next, your care continues to move forward between sessions as new information emerges and we learn what works best for you.

Continuing Your Care Beyond the Program

The six-week program is designed as an intensive first phase—not necessarily the end of our work together. Once that foundation is in place, you may no longer need the same level of intensive support. At the conclusion of the program, we’ll reassess your progress and decide what level of ongoing support makes sense for you.

After completing Better Numbers, you’ll have the option to transition to the High-Support Continuity Plan if you would still benefit from additional support between appointments. This lower-intensity monthly option provides continued review and refinement of your existing plan, personalized tools and resources as new needs arise, and between-session coaching and support. The base plan is $99/month, plus $25/month when you have 1–2 follow-up sessions or $50/month when you have 3–4 follow-up sessions. Your level can change from month to month as your needs evolve.

Or, you can simply transition to insurance-covered follow-up sessions. With this option, our work is focused primarily within your scheduled appointments, without the additional between-session review, personalized work, and the type of ongoing coaching provided through the Continuity Plan.

There is no need to decide now. At the end of the six weeks, we’ll determine together which level of support best fits where you are and what you need next.
